ExportLayerToFiles with layer group naming

Someone asked to have the Adobe 'Export Layers To Files' script modified so that if a layer is in a layer group that group name is added to the layer name.

Tested 2 groups deep. Note it's now up to the user to ensure that the layers/groups are unique as this version no longer does the zero pad.
